Immunizations

It is state law that all students have the following immunizations on file in the school that they attend.

• DPT/DT/TD - At least five doses required unless 4th dose after 4th birthday (if 7years or older only 3 doses required), one dose MUST be after 4th birthday, then one dose every ten years.

• OPV/IPV - At least 4 doses unless 3rd dose after 4th birthday, one dose MUST be after 4th birthday.

• MEASLES - 2dose after 1st birthday

• MUMPS - One dose after 1st birthday.

• RUBELLA - One dose after 1st birthday.

• HEPATITIS B - 3 doses required

• VARIVAX (Chickenpox) - required after 1st birthday OR a written statement from parent that student has had chickenpox disease
